Conduct an occupant satisfaction survey for all occupants
Rationale: Regularly fielding an occupant satisfaction survey and ensuring a sufficient response rate provides insight into occupant perceptions of site design and operations, which highlights opportunities to improve features and programs to support mental, physical, and social health.

Requirements
Demonstrate compliance with the option below:
Occupant Satisfaction Survey (Full Credit):
Demonstrate compliance with all of the following requirements:
Low-Rise Residential with shared interior spaces (LR+), Low-Rise Residential with no shared interior spaces (LR)
- The occupant satisfaction survey is fielded annually.
- The occupant satisfaction survey includes a summary of a minimum of four of the following categories:
- commuter infrastructure (e.g. bicycle parking, showers and lockers, transit stops)
- programming
- building access
- outdoor spaces
- shared spaces
- workplaces
- food access
Meets the response rate below based on the number of residential occupants within the project over the age of 18.
- for projects with 1 - 1,000 residential occupants, a 30% response rate
- for projects with 1,001 - 2,000 residential occupants, a 25% response rate
- for projects with 2,001- 3,500 residential occupants, a 20% response rate
- for projects with 3,501 - 5,000 residential occupants, a 15% response rate
- for projects with 5,001 - 10,000 residential occupants, a 10% response rate
- for projects with over 10,000 residential occupants, the required response rate decreases by 1% for every additional 1,000 occupants to a minimum of 5%.
Note that response rates must account for the total number of residents within the entire project.
- Implement interventions related to the occupant satisfaction survey results annually through a minimum of one of the following:
- design
- policy
- programming
- operations.

- Checkbox to certify that the occupant satisfaction survey will be fielded annually.
- Requirement 2:
- Checkbox to certify that the occupant satisfaction survey will include a detailed summary detailing a minimum of four of the following (minimum four):

- Upload a copy of the fielded occupant satisfaction survey detailing the results for the selected categories.
- Checkbox to certify that the occupant satisfaction survey includes a detailed summary detailing a minimum of four of the following (minimum four):
- Requirement 3:
- Text box to input the total number of residential occupants over the age of 18 within the project.
- Text box to input the number of survey responses that were received.
- Requirement 4:
- Checkbox to certify that interventions related to the occupant satisfaction survey results are implemented annually.
- Upload a description of the most recent interventions implemented based on the occupant satisfaction survey results.
